Records Requests require up to 72 hours (3 business days) to process after the completed request form and a valid government-issued identification have been received and verified.

Clients may print and complete the Records Request Form and bring it to the IOP office in Mesa. Upon submission, the Site Manager will verify the client's identification and email the completed form, along with verification of the client's ID, to the Executive Director and Clinical Director for review and approval.

Once the request has been processed, the client will be notified by email when their records are ready for pick up.
